Things You Will Need to Consider Before Breast Revision Surgery

1. What type of breast implant do you want? Will you want to change from saline to silicon or vice versa? 2. Where do you want your breast implants placed, above or below the muscle? There are pros and cons for each placement. Dr. Delgado prefers under the muscle as he feels it gives a more natural look. The muscle is less likely to interfere with mammograms. Additionally, there is less rippling for women who do not have much breast tissue. However, Dr. Delgado will want to discuss which placement is best for each patient.

3. There needs to be a discussion for the size of breast implants. The size of breast implants is very important and causes a lot of concern for many women. Many women can choose larger or smaller implants. However, there are limitations that are unique to each woman. Dr. Delgado will be able to advise you your choices based on your goals.

4. Is a breast lift going to be necessary to get optimal results? As the breasts change due to aging, or weight fluctuations, a breast lift may be needed to remove excess skin and reposition the nipple higher on the breast mound.

Hopefully, when you had your initial consultation with your breast augmentation surgeon you were advised that at some time in the future, usually after ten years, you may need to have breast revision surgery, possibly replacing your breast implants.

Variations of Breast Revision Surgery

The reason for breast revision surgery will determine how the procedure will be done.

• Capsular Contracture: The surgeon most likely will use the same incision location as the original breast augmentation surgery. The hardened capsule will be removed during breast revision surgery, and the implants will be replaced.
• Mal-positioned Breast Implants: Sometimes the breast implant pockets may be placed too close together or too far apart, to correct this, the surgeon will reconstruct the pocket by manipulating the capsule’s scar tissue that surrounds the implant. Then the surgeon will use suturing techniques to get the implant into the appropriate position.
• Rippling Breast Implants: Rippling may be caused by saline implants and sometimes the edges of the implants can be felt. To correct the rippling, the implants may be repositioned to under the muscle or changing to silicone implants or both.
• Changing Breast Implant Size: If a larger implant is desired, the surgeon will need to make the pocket bigger. For smaller implants, the surgeon may reduce the size of the pockets with sutures. Sometimes, Dr. Miguel Delgado, M.D. will suggest that a breast lift may be needed when the pockets are reduced or permanently removed.

Most breast implant manufacturers give a lifetime guarantee on their implants if they should fail. The manufacturers will supply new breast implants for surgery, but they only give 10 years for additional financial aid to help offset surgery costs. Statistics show that on average women may need a revision after ten years.

Depending on the reason for breast revision determines when it should be done. Before and After Surgery Instructions for Breast Revision Patients

Patient Participation is a Must

There are a lot of instructions for patients, both before surgery and after, therefore Dr. Delgado designed a comprehensive booklet for patients to have on hand that they and their caregiver can refer to.

The booklet, which is almost 40 pages in length, is divided into several sections including a table of contents for easy reference. In the first chapter, which is titled “Starting Now,” the patient is advised to stop smoking. Start taking a multi-vitamin, and get lab tests done. When it is two weeks before surgery, Miguel Delgado, M.D. warns his patients to stop taking any products that contain blood thinners such as aspirin. However, aspirin is not the only culprit that thins the blood. In the booklet, there are three pages of medications, vitamins and supplements that must be stopped as they all contribute to thinning of the blood, which can cause complications during and after all surgeries, including gynecomastia, face lifts and mommy makeovers.

Critical to Avoid Blood Thinners

Many people are not aware of many items that thin the blood, such as; Alka Seltzer, Motrin, Vitamin E, Fish Oil, St. John’s Wart, Ginkgo Biloba, just to name a few. To be safe, Dr. Delgado wants his patients to be sure and let him know of any and all supplements and medications taken on a regular basis.

In the second section which is titled “The Day Before Surgery,” patients are reminded to get all prescriptions filled, guidelines for eating, drinking and bathing are reviewed and the surgery time is confirmed.

As this website describes, there are several different causes for breast revision surgery. When the reason for breast revision surgery is due to ruptured implants the implants and scar tissue should be removed as soon as possible so that the breast implant pocket is not damaged. Otherwise, as long as the patient is not experiencing any discomfort or redness, breast revision can be scheduled at the convenience of the patient.

Breast Augmentation is Just the Beginning

Every woman is different; there is no way to know how many breast revision surgeries may be needed. There is the rare woman, who may not ever need a revision, but most will need at least one, and many may need two or more. The Dangers of Smoking for the Plastic Surgery Breast Revision Patient

Almost 1 in 5 Americans smoke. It is not surprising that Miguel Delgado, M.D. is frequently asked by his breast revision patients, “How important is it to quit smoking before surgery?” His reply is always, “Extremely important, I can’t emphasize enough the dangers that smoking can cause for a cosmetic plastic surgery patient.”

Plastic surgery changes the shape of different areas of the body; this is achieved by moving skin and tissue around and thereby altering the blood supply. Cigarette smoke contains over 250 known toxins, besides nicotine; there is carbon monoxide, nitric oxide, and hydrogen cyanide, all of which compromise wound healing. The blood vessels bring oxygen to the tissues, in addition to healing aspects; the oxygenated blood fights infection and delivers important medication such as antibiotics.

Nicotine affects blood flow because the blood vessels constrict, and then the blood may clot more easily clogging small blood vessels and capillaries. This clogging could result in a reduction of blood to the tissues causing necrosis, which is the death of cells in living tissue. When this happens, the surrounding tissues become inflamed requiring surgical removal called debridement. When this type of surgery becomes necessary, the outcome of the cosmetic surgical procedure may be compromised.

Medical Studies Report the Risks of Smoking

The American Society of Anesthesiologists reported a study that shows surgery results between smokers and non-smokers:

• Smokers 50% more likely to develop pneumonia
• Smokers 57% more likely to have cardiac arrest
• Smokers 73% more likely to have a stroke
• Smokers 80% more likely to have a heart attack

In another recent study by The European Society of Anesthesiology, smokers need 33% more anesthesia throughout surgery and 23% more pain medication. Surprisingly, those exposed to second-hand smoke needed 20% more anesthesia and 18% more pain medication.

Addiction to cigarettes should not to be taken lightly. Most people who smoke wish they could quit, but addiction to nicotine can be an uncontrollable dependence causing severe mental, emotional and even physical reactions for someone trying to kick the habit.

Some patients may be tempted not to disclose their smoking status to their surgeon. Preoperative testing can show if nicotine is present in the blood and thereby affecting the safety of surgery. Recovery for Breast Revision Surgery

 47-year-old mother of two and resident of San Francisco, California who presents to the Marin County office for breast revision surgery. She underwent a vertical breast lift with normal saline breasts implants approximately 10 years ago.  She underwent a second vertical breast lift with removal of the normal saline implants and placement of 360cc high-profile Mentor silicone breast implants. Her photographs are seen approximately six months after surgery. Her surgery was performed at Dr. Delgado’s private Marin Cosmetic Surgery Center in Novato, Marin County, California.

Breast Revision at Least Once

Miguel Delgado, M.D. emphasizes to all his breast augmentation patients that they will most likely need breast revision surgery at least once in their life. For many women, more than one breast revision surgery may be needed. There are many different reasons why breast revision may be warranted, and they are explained in detail on this website.

The recovery can vary depending on the type of revision surgery that is to be performed. If a woman is just having her implants removed, generally only minor discomfort is experienced. Over the counter analgesics are usually all that are needed. However, if a woman is having a breast lift done in conjunction with breast implant removal, recovery will be longer. There will be possible numbness and tingling resulting, but usually lasts only a few weeks.

Important Factors, Technique and Skill

For breast implant replacement, recovery may be about the same as the original breast augmentation surgery. The degree of discomfort will be dependent on the size and placement of the implants, and of course, the skill of the surgeon.

Most likely a compression bra is to be worn for 2 weeks or more. The bra will help prevent the accumulation of fluid, help control swelling, and keep the implants in place. Sleeping elevated is also recommended to help with swelling.

Follow Surgeon’s Instructions

Among the post-operative instructions, patients will be advised to stop all blood thinning products, vitamins, herbs, and medications. An extensive list is in the booklet of what should be avoided. It is a good idea to bring in a list of any and all medications and supplements that you take at your pre-operative visit.
Patients will need to arrange to have a friend or family member take them home after surgery and look after them for the first 24 hours. If drains have been placed to remove extra fluid buildup, the patient must wait until they are removed before bathing, to avoid possible infection. However, sponge baths are allowed.

Before resuming strenuous exercise or any heavy lifting, you need to get clearance from your surgeon, usually within a few weeks.
